Partners for Birth to Three

History


In compliance with IDEA’s Part C, LEARN’s Partners for Birth to Three program has been providing critical services to over 2,000 families through a trans-disciplinary model since 1996. We complete approximately 30 eligibility evaluations each month and provide services for about 100 children and their families.  Family support is the hallmark of our service delivery. Our providers respect and encourage family choice in services and programs and services are provided in conjunction with families using a coaching model.

Staffing


Our service providers are professionals who have received special training in supporting families with infants and young children. Early Interventionists strive to provide services in a "natural" environment, that is, in places where other infants and toddlers and their families are found. For most of the families, this is in the family's home.

With increasing frequency though, services are provided at alternative sites such as child care centers, at play groups held by local Youth Service Bureaus, in the play area at a McDonald's, in local parks, at a swimming pool or at a mall. The goal is to give families the information and assistance needed to enhance social, language, motor or cognitive skills of children in their daily routines.

Services are provided by LEARN staff and community individuals and agencies to assure that comprehensive services are available to families in Birth to Three. Those contracts include:

Service Delivery


Each child and family receives a comprehensive assessment. After the parents have selected a program, they and a team of professionals who match their child's needs will develop an Individualized Family Service Plan. This plan will describe the child's strengths and needs as well as the family's concerns and priorities for their child. It will also detail what services and supports need to be provided including their location and frequency.
